Certain forms are needed in the claims process to aid in our investigation, to provide information and to complete settlements. To expedite the claims process we have made some of the required forms available on our Online Claims Service Center.

Our claims representatives will assist you with any questions and the completion of the necessary forms during the claims process. These forms are intended for the sole use of Indiana Farmers Mutual customers and the resolution of their claims. Use of these forms outside the intended scope is prohibited.

You can print these forms directly from our web site alleviating the time required for us to mail or fax the forms to you. Some of the forms can be completed and emailed to your assigned claims representative, others require your original signature and will need to be mailed and/or faxed to our office.
